You can find their Block and Lot information --- which you will need for the form --- here:

Your parcel number will look like this: #-###-##.  The first digit represents the borough, the middle number is the block, and the last number is the lot.

I have received confirmation from the Department of Health that only one notification form per location is required even if you have more than one hive at that location:

"If your two hives are at the same rooftop location you do not need to submit two forms.  Only if the hives are at different locations, you would need to submit two forms indicating the location of each."

The form has been revised with a box to indicate the number of hives.

Please remember to register your hives with NYC if you have not already! 

You should also register your hives with NY (State).
