Missing bees...

My first year hive is all but disappeared! there are a small handful of bees remaining in the hive, so I'm not sure if they all died or perhaps left to find a different home. I use a top feeder and when I checked it this morning it was full of tiny ants and some mold. I had checked it less than a week ago and it looked fine, but I guess I should be checking more often?

Any thoughts on where my bees might be, or what I might have done wrong?
